How to Obtain IDQ Questionnaires for Your Class

To generate your questionnaires, you must use the Instructor Designed Questionnaire Requisition form. Photocopies of this form may not be used. Original forms of this Requisition may be obtained without charge by contacting TSQS and asking for the IDQ Requisition form. A separate Requisition form must be completed for each different questionnaire. If several instructors wish to use the same set of questions or if the same questionnaire is to be administered to several different classes, such multiple requests may be communicated by the use of a single IDQ Requisition form along with a Class Identification Attachment.

Please allow at least two weeks for the generation of your IDQ questionnaires. If you intend to employ the campus mail system for sending the requisition and receiving the questionnaires, you should submit the Requisition form at least three weeks in advance of the time you wish to have possession of the questionnaires. Budgeting this amount of time is particularly important for evaluations occurring in November and March.
 
Consult the relevant catalog to decide which questions you wish to have appear on your questionnaire. You may select any of the catalog's questions and/or include your own unique questions.

The total number of questions that may be requested on the generic IDQ form may not exceed 42. (There are additional restrictions for the GFC catalog when the special Universal Student Ratings of Instruction form is used.) Of the 42, no more than 40 can be rating-type questions (questions with catalog numbers below 900 or unique questions that have been worded as rating-type questions). It is not permissible to include more than 5 open-ended questions (catalog numbers above 900). Note that if you wish to have 5 open-ended questions, the overall maximum of 42 restricts you to no more than 37 rating-type questions.

Complete the IDQ Requisition form as follows:

  1. Billing Information: For billing purposes, indicate your PeopleSoft Speedcode and a COA account number. The combination of these two values is a 11-character value which can be coded in the Account Number grid if the characters are all numeric. If your Speedcode contains alphabetic characters, write the 5-character code in the first 5 boxes but code zeros in the bubbles of the corresponding columns. The COA account number will usually be 502306.

    (The charges assessed for producing and processing GFC questionnaires will be billed to the Vice-President's office. Therefore, please do not include any Billing Information on the Requisition form for requisitions using the GFC catalog except when your request falls outside the basic service provided by the GFC Policy for Universal Students' Ratings of Instruction.)

  2. Identification of Requisitioner: At the top-center of the form, fill in the requested information to identify the person submitting this requisition. This is the person contacted by CNS if there are problems with the requisition. Normally, this will also be the person to whom the generated questionnaires will be sent. Space is provided to indicate more specific instructions for the delivery of the questionnaires.
  3. IDQ Catalog Identifier: Please fill in the circle adjacent to the name of the catalog that you used to select your questions. If you used a catalog that is not listed on the Requisition form, print the name of the catalog on the line provided for "Other".
  4. Item Requisition Grid: Enter the catalog numbers and/or block-ids corresponding to the questions that you wish to have printed on the questionnaire. The questions will be printed in the sequence that you request them, provided that different types of questions appear in the following order:
    1. questions with catalog numbers below 900 and unique rating-type questions (note that an earlier restriction that unique questions will follow cataloged questions has been removed);
    2. questions being repeated for team-teaching assessment;
    3. questions with catalog numbers 900 or above and unique questions in open-ended format.
  5. Instructor Information: Print the instructor's name, course, and section as it should appear on the questionnaires and reports. In addition, the mailing address is needed so that the questionnaires and reports can be delivered to the right person. If the same questionnaire is to be used for multiple instructors and/or classes, the Instructor Identification information for each class should be provided on a separate sheet of paper using the format indicated on the Class Identification Attachment.
  6. Number of Questionnaires required: Since this is a custom-designed form, request only the number that will likely be needed for the particular class, i.e., for the particular group of students to which the questionnaire will be administered on a specific occasion.
  7. For Reports, select Reference Groups as follows: Three menu-lines are provided for selection of reference groups. Please fill in the circles which indicate the manner in which you would like the reference groups selected. You may choose one or more major groupings, i.e.,
    1. all courses,
    2. courses in your faculty, or
    3. courses in your department.

    Within each of these groupings, you may further restrict the reference group to consist of only classes having a similar class size and/or a similar course level. Note that, currently, reference to faculties and departments is only possible for the GFC and CITL catalogs. Prior arrangements must be made with CNS before this feature can be used with one of the other catalogs.

  8. Delivery of Reports: An option is provided to indicate the individual(s) who should receive the report at the completion of data collection and computer processing. If you indicate that the report is to be delivered to Respective Addressees, the report will be mailed directly to the appropriate instructor(s). The processed questionnaires will be returned along with these reports.
  9. Date Required:
    • Indicate to the left of Forms Generation the date by which you require the questionnaires.
    • Indicate to the left of Scanning & Reporting the date when you desire to receive the report. Normally, the report is sent out after students' final marks are posted.
  10. Unique-Question and Class-Identification Attachments: Please indicate the number of attachment pages accompanying this Requisition so that we can be confident that the complete request has been received.

Submitting the IDQ Requisition form:

  • Remove and retain for your reference the last copy of the Requisition form--labelled Job Submission Receipt near the bottom-center of the form.
  • Staple the attachment pages, if any, to the stub-portion of the Requisition form.
  • Please do not fold the Requisition form.
  • Forward the completed Requisition form to Test Scoring & Questionnaire Services, CNS.

Submitting the Questionnaires for Scanning:

It is important to return the completed questionnaires to TSQS as soon as possible and to allow time for the processing of questionaires from all instructors before asking that the reports be produced and returned as requested. Since each questionnaire has the original Requisition Number printed on it and the original form will have been retained by TSQS, it is not necessary to submit a Requisition form with the completed questionnaires. However, please ensure that questionnaires from several requisitions are not mixed together when delivering them to CNS/TSQS for processing.
