the Questionnaires for Scanning:
It is important
to return the completed questionnaires to TSQS
as soon as possible and to allow time for the processing of questionaires
from all instructors before asking that the reports be produced and returned
as requested. Since each questionnaire has the original Requisition Number
printed on it and the original form will have been retained by TSQS, it
is not necessary to submit a Requisition form with the completed questionnaires.
However, please ensure that questionnaires from several requisitions are
not mixed together when delivering them to CNS/TSQS for processing.
If any questions arise about the processing of your IDQ Requisition, please be prepared to reference the Requisition No. printed at the top of your Job Submission Receipt when you phone TSQS.
