Student Mask Exemption Application

Following restrictions in regard to the COVID-19 pandemic enacted by Alberta Health Services and the Government of Alberta, we ask that all students review the University of Alberta’s Student Mask Exemption Procedure. Important information related to when and where masks must be worn on University of Alberta campuses, the criteria under which students may be exempted from wearing masks, and more is outlined in this procedure document. 

The following students are exempt from the requirement to wear a mask. Any student who is:

  • unable to place, use, or remove a face covering without assistance;
  • unable to wear a mask or other face-covering due to a mental or physical concern or limitation, or any other protected ground under the Alberta Human Rights Act; and those who are
  • providing assistance or care to someone with a disability who would be hindered in providing that assistance or care by the wearing of a face mask.

Students who meet any of these criteria may apply for an exemption from wearing masks on University of Alberta campuses by completing the Student Mask Exemption Application. Learn more about and access the Application below.

Please consider the following information if you need to apply for a student mask exemption: 

  • If you need to access the application in an alternate format or have questions, please email studentmask@ualberta.ca.
  • You may only identify one exemption criteria in your application. If you meet multiple student mask exemption criteria, please complete your application based on the primary criteria that impacts your ability to wear a mask or other face covering.
  • You are NOT approved for a mask exemption until you receive written notification of your exemption approval. 
  • Approved student mask exemptions may include conditions or limitations.
  • If you are a University of Alberta staff member or instructor who needs to confirm an approved student mask exemption, you may request confirmation of approved student mask exemptions
  • If you are a University of Alberta staff member or instructor who needs to apply for your own mask exemption, please consult the Employee Mask Exemption Procedure.
Application Instructions

To complete the Student Mask Exemption Application, you will need to: 

  • Provide your University of Alberta Campus Computing ID (CCID) and Student ID Number.
  • Plan to spend about 5 to 10 minutes completing the form.
  • Give the most appropriate response to the questions based on your circumstances.
  • If you are unable to place, use, or remove a face covering without assistance, you must provide a PDF version of medical documentation (completed by a qualified healthcare practitioner) that explains in detail why you are not able to place, use, or remove a face covering without assistance. 
  • If you are unable to wear a mask or other face covering due to a mental or physical concern or limitation as a protected ground under the Alberta Human Rights Act, you must provide a PDF version of medical documentation (completed by a qualified healthcare practitioner) that explains in detail why you are unable to wear a mask or other face covering due to a mental or physical concern or limitation. 
  • If you are unable to wear a mask or other face covering due to a protected ground other than a mental or physical concern or limitation under the Alberta Human Rights Act, you must explain in detail:
    • the protected ground that makes you unable to wear a mask or other face covering; and,
    • how and why this protected ground makes you unable to wear a mask or other face covering.
  • If you are providing assistance or care to someone with a disability who would be hindered in providing that assistance or care by the wearing of a face mask, you must explain in detail:
    • your relationship to the individual to whom you are providing assistance or care (e.g., parent, caregiver of sibling or parent, caregiver of friend, paid personal care attendant provider); and
    • how and why the assistance you provide hinders the use of a mask. 
Documentation Requirements
  • In the application form, you will be prompted to upload your documentation.
  • Documents that are not clearly readable will not be accepted. 
  • If you do not have a scanner or cannot access electronic copies of your documentation, you can use a smartphone to take a picture of each individual page of your documentation and convert the images into a PDF document using applications/programs such as Adobe, iScanner, or Cam Scanner.
  • Use the following format to name all PDFs: last name_first initial_yyyymmdd (e.g., Smith_J_20200125). Important note: use the date on which the documentation was completed by your health practitioner (as listed on the documentation) in your file name.
  • All fields outlined in the form are required to be completed.
  • A copy of your completed form will be emailed to you.

Important considerations when completing your form:

  • An incomplete form may delay the review of your submission. 
  • Once you submit your form, a staff member may contact you within 7 working days to book a meeting to discuss your application, accommodation needs, and options for mask wearing.
  • You may not apply on behalf of other individuals.
  • Check your University of Alberta email account frequently for communication about your application.

Please note that you are NOT approved for a mask exemption until you receive written notification of your exemption approval. 

Application Outcomes & Next Steps

Notification of the Application Decision
Student Mask Exemption Applications will be reviewed and either approved or denied. Approvals may include conditions or limitations of your mask exemption. Decisions will be emailed to your University of Alberta email account within 14 business days of your application submission. Please check your account frequently for communication about your application.

If your application is approved and you require confirmation of the approval
University of Alberta staff members or instructors may request confirmation of approved student mask exemptions here. Please note that we will only confirm your approval for a mask exemption (including any conditions or limitations of your mask exemption). We will not release any personal information included in your application.

If your application is approved but the exemption criteria on which you were approved later changes (e.g., your medical condition improves, you no longer assist or provide care to an individual with a disability)
You must notify us by email at studentmask@ualberta.ca that your eligibility for a mask exemption is no longer valid. Upon receipt of this notification, we will cancel your mask exemption. 

If your application is denied
You may request a review of your application decision. To request a review of your application decision, email studentmask@ualberta.ca within 14 business days of receiving your application decision. Please use the email subject line “Decision Review” and briefly explain why you think the decision should be reconsidered.

If your application is denied and you later obtain new documentation supporting your eligibility for an exemption from wearing a mask
You may submit a new application.
