Completing your Annual Report

Changes to the Research section of the Annual Report Form were approved at Faculty Evaluation Committee on February 15, 2014, and Engineering Faculty Council on May 22, 2014. The changes will comprise the addition of self-selected research areas, standardized keywords, and user-chosen keywords. This data will be used to maintain a hierarchical database of research activities on the faculty’s web site which will be used to help connect faculty members to external organizations seeking collaborators, to potential graduate students and to companies or media seeking specific expertise.

Access to the web-based Annual Report Form and system will open July 1, 2018.

Please complete the web-based form for the period September 1, 2017 to August 31, 2018, print the PDF version, and submit your Annual Report to your Department Chair on or before September 15, 2018.

Conflict of Interest Form

In accordance with Engineering Faculty Council procedures, please complete Conflict of Interest information for the period July 1, 2018 – June 30, 2019. We have replaced the Conflict of Interest template with a web-based form which you can access from the personal data section on the annual report form. The conflict of interest form will print at the end of the annual report. Please review the data, sign the form, and submit it with your Annual Report.
