Health Spending Account Reimbursement of additional health and dental expenses not eligible under the employee’s other benefit plans. Annual allocation of $750 per calendar year per staff member. Credit carry-forward for one year of unused allocation. Date of appointment Paid by the University
Employee and Family Assistance Program (EFAP) Comprehensive psychological counseling, wellness and work life services (e.g. legal and financial consultations). Human Solutions is the service provider for the University Date of appointment Paid by the University
Long Term Disability Loss of income in the event of a long-term disability Benefit is 70% of salary and is taxable. The University’s Health Promotion and Work Life Services unit initiates the claim process. Application for benefits occurs after 26 weeks elimination period. Coverage ceases end of the month in which employee turns 65. Date of appointment Paid by the University
Life Insurance Basic Provides a lump sum to the designated beneficiary(ies) in the event of death $100,000 flat coverage amount. The University’s Human Resource Services unit would initiate and submit the claim on behalf of the beneficiary(ies). Date of appointment Paid by the University
Optional Life  Additional life insurance can be purchased in units of $10,000 up to $500,000. Evidence of Insurability required for amounts over $180,000 or on total principal if late applicant. An application is required. Date of appointment Employee Paid

Spousal Critical Illness Spousal critical illness insurance can be purchased in increments of $25,000 to a maximum of $300,000. Lump sum benefit paid to the employee if the spouse is diagnosed with a covered disease. Accidental Death & Dismemberment Additional insurance in the event of accidental death or a permanent dismemberment. Application required. Employee or family coverage is available. Business Travel  Provides additional coverage for accidental death or bodily injury which occurs while traveling on business for the University. Principal Sum of $100,000 Date of appointment Paid by the University
Occupational Accidental Death and Dismemberment  Provides insurance coverage in the event you suffer accidental death or dismemberment as a result of an accident which occurs while you are at work. Principal Sum of $25,000. Date of appointment Paid by the University
Universities Academic Pension Plan (UAPP) Defined benefit pension plan which provides retirement and survivor income replacement. Date of appointment Both the University and the Employee make equal contributions to the plan.
Executive Defined Contribution Supplemental Retirement Plan (EDCSRP)  A defined contribution retirement benefit in addition to the UAPP for members with annual pensionable salary exceeding the annual UAPP salary cap. Date of appointment. Subject to annual pensionable salary exceeding the UAPP pensionable salary cap to the EDCSRP salary cap  100% employer paid contribution based on sum of member age and pensionable service
Medical Leave Covers illness or injury to a maximum of 26 weeks. Medical Certificate required. Date of Appointment  100% of salary
Vacation Leave Departmental procedures apply Earned from date of appointment 22 working days per annum.
Personal Leave Program Voluntary unpaid leave days under a special program. (This program is subject to review annually) Upon employee request 5 days. Personal Leave levy of 1.92% to be applied from July 2012 to June 2013
Childbirth Leave Up to 15 weeks. Medical certificate and proof of Employment Insurance payment is required for “top up” payments. Date of appointment Must be eligible for Employment Insurance maternity benefits for “top up” 95% of salary "top up" on Employment Insurance payments
Parental Leave Up to 37 weeks. Proof of Employment Insurance payment is required to be eligible for 10 week “top up” Date of appointment Must be eligible for Employment Insurance parent leave benefits for “top up” 95% of salary "top up" on Employment Insurance payments for 10 weeks
Physical Education, Recreation Facilities Fitness, swimming, squash, racquetball, etc.  Date of appointment This does not include classes. Obtain a staff card for unlimited access at the ONECard office
Remission of Tuition Credit courses. Academic Remission of Tuition Fees form is submitted to Financial Services. Date of appointment Equivalent of 4 single term Arts courses per year
Child Care Benefit Funds provided for the reimbursement of child care expenses incurred by staff member. Date of appointment 50% reimbursement of expenses up to a maximum of $2,000 per calendar year per eligible child
Professional Expense Reimbursement (PER) Funds provided for the reimbursement of professional expenses incurred by staff member. Expense claims are submitted to Human Resource Services. Date of appointment $1,400 per academic year
Compassionate Care and Emergency Leave Up to 6 months of paid or unpaid leave in cases of catastrophic illness or where an urgent domestic situation requiring care and support of a family member. Date of appointment 100% of Salary
Personal Leave Days Personal leave days are made available for the purposes of conducting personal business and meeting family responsibilities including attending appointments with family members and/or dependents. Date of appointment Allocation of five days per year
