Academic Staff Checklist

If you are an Academic Staff Member experiencing health concerns that are impacting your ability to maintain work, or, if you have been on Medical Leave and absent from duties for twenty (20) days or if you expect that the medical leave will result in an absence of more than twenty (20) days, your responsibilities as a staff member are to:

  1. Notify your Department Chair (or designate) of your absence immediately and your expected duration of your medical leave.
  2. Maintain contact with your Manager during your leave and update your Manager as to your prognosis for recovery and expected return to work.
    As a University of Alberta staff member, during a Medical Leave you are eligible for health recovery and return to work support services from Homewood Health Inc. (HHI) and HRS/Organizational Development, Equity and Health (ODEH).

  3. Download and have your treating physician complete the Medical Certificate form. Please fax, or have your physician’s office fax the completed Medical Certificate form to HHI at 780-429-1747. 
    The University of Alberta will cover the costs of the Medical Certificate reporting fee. If there is a cost, please have your doctor’s office fax their invoice directly to HHI at 780-429-1747.

    We recommend faxing medical documents to HHI, however if you choose to scan and email information, Please note that the University's Information and Privacy Office (IPO) does not recommend that medical information be sent by email. Please consider these risks and alternatives when you share medical and other personal information in the course of accessing services and resources to assist with your illness, injury or disability.
  4. Be Available by phone or email. Your Homewood Health Support Consultant (HSC) will contact you typically within 24-48 hours of receiving notification of your absence.
  5. Discuss the Consent for Release of Health Information form with your HSC.
  6. Regularly communicate with your HSC and your immediate supervisor during the duration of your leave.
  7. Provide updated medical information as required.
  8. Participate in return to work planning and return to work as soon as it is safe and healthy for you to do so.
