Support Staff Roles and Responsibilities While on General Illness

We are accountable for taking responsibility for our own health and well-being, however, we may experience an illness/injury which requires us to be away from work for a time. Should this occur,if your illness/injury is known initially to be for more than 10 working days or the illness continues for more than 10 working days, your responsibilities as an employee are:

  • Notifying your Supervisor/Manager of your absence immediately and providing your expected return to work date if known.
  • Providing medical documentation to support proof of illness. As per Article 16.10 of the Non-Academic Staff Association Agreement, it is your responsibility to provide proof of illness to support General Illness Benefits.

- Fax your completed Medical to Support General Illness Benefits form to Homewood Health at 780-429-1747.
- Your physicians office may fax the Medical to Support General Illness Benefits form directly to Homewood Health Inc. (HHI) at 780-429-1747.
- The University of Alberta will cover the costs of the medical reporting fee. Please have your doctor's office fax their invoice directly to HHI at 780-429-1747.

Please consider these risks and alternatives when you share medical and other personal information in the course of accessing services and resources to assist with your illness, injury or disability.

  • Maintaining contact with your Supervisor/Manager/HSC during your absence and updating your Supervisor/ Manager / HSC as to your prognosis for recovery and expected return to work.
  • Actively participating in your recovery by collaborating with your treatment providers and your HHI Case Coordinator who will assist both you and your treatment team in identifying the best resources and access to rehabilitative or other services and assist you in a safe, healthy, productive and sustainable return to work.
  • Attending appointments with specialists and rehabilitation providers as arranged and following all usual and customary treatment/activities recommended by care providers.
  • Continuing to provide objective medical information throughout the duration of your illness.
  • Advising your Supervisor/Manager and ODEH Return to Work Consultant (RTWC) when you are deemed able to return to full or modified or alternate duties/hours.
  • Engaging in return-to-work discussions with your Supervisor/Manager and RTWC on alternatives that will support a timely and sustainable return to work.
  • Participating in modified or alternate work assignments within any limitations/restrictions approved by a physician.
  • Consulting with a Union Representative if you have concerns with your ability to effectively represent yourself.
  • Complying with the terms of relevant Agreements or Benefits Plans, policies and procedures including completion of the necessary documentation and/or obtaining documentation from their physician as required.
  • Advising your RTWC and Supervisor/Manager of any problems associated with a modified or alternate work assignment.
  • Advising your supervisor/manager in advance of planned absences such as scheduled surgery, as soon as you become aware.
