Important Message About Immunization Forms
Effective Tuesday, January 17, 2017, a new immunization form - AFMC Student Portal Immunization & Testing Form - has been launched at all seventeen AFMC member schools. This form will be used for immunization and testing verification. No pre-existing forms can be completed after January 16, 2017. Old immunization forms already in process will only be accepted until February 28, 2017. As of March 1, 2017, students must submit this NEW form for all applications. Students who have not started the application process before January 17 must use the new common immunization form. For forms that are not available at the time of application submission, (ie. third dose of polio has not been completed yet) applications will be opened up to allow students to submit an updated form once it has been completed. This will not delay processing times or placements in que, however all students will need to ensure this form has been completed and submitted no later than 2 weeks prior to their elective start date. Message re: Mask-Fitting
As a result of a policy change in Alberta Health Services - the province wide health-care provider which includes Edmonton and area hospitals - visiting students scheduled to attend these sites must be fitted with a face mask that meets the N95 standard, using 3M company products.
Each student engaging in a teaching-hospital placement is now required to carry evidence that this requirement has been met.
It is expected that students from outside Edmonton will arrange to meet this standard within their local university according to guidelines set by their Occupational Health and Safety Office.
