SAGE Health Services Partnership Thriving

Under leadership of NPs Anne Summach, Lily Lo, and Tammy O'Rourke, SAGE Health Services partnership Brings NP-Led care directly to thousands of seniors.

As of March 1st 2019, the University of Alberta Faculty of Nursing partnership with health services at Sage Seniors' Association successfully completed one year of operations, during which they booked 2,487 appointments, served 640 clients, and rostered 276 of these clients.

Throughout this year, Anne Summach, Lily Lo, and Tammy O'Rourke have developed many meaningful and beneficial partnerships; mentored numerous RN, NP, and OT students; and delivered health presentations to community agencies, senior residences, health forums, and health care providers.

They've also implemented a bus program to address mobility and social isolation issues that seniors experience, and worked across professions to incorporate a dental hygienist, denturist, optometrist, foot care provider, and a pharmacist into their service delivery-a truly patient-centred approach to care.

With ongoing support, we are excited to see what the future has in store for this first-of-its-kind partnership.
