The forms below are to be used for current Faculty of Science students who are actively enrolled in their programs.
Degree Assessment Reports can now be accessed through your Bear tracks account. Select "Academic Requirements" in the drop down menu under "Academics".
Academic Forms

Program Change Forms
(Available August 1 - October 31 and Jan 1 - April 30)
- Subject Area Change Request (For students currently in BSc General, BSc Major, BSc Honors)
- Subject Area Change Request- BSc/BEd (For students currently in a BSc Science/Education Combined Degree)
The following program changes require a formal application through the U of A admission portal (October 1 - March 30):

Transfer from the BSc Major degree to BSc Honors degree |

Transfer from the BSc Honors degree to BSc Major degree |
|
|
Transfer into or out of the BSc/BEd combined degree |
|
|
Transfer from one of the discontinued degrees to one of the new degrees |
Deferred Final Exam Forms (Make sure you qualify. Read the calendar)

*It is important for you to fill out the deferred exam form accurately and completely. Failure to do so may result in a delayed decision or a denied application. Once the evaluation has been completed and a decision has been made you will be notified.

Note: The application for graduation is in your Beartracks account (under Graduation in Academic Records)
- Request to Add an Embedded Certificate(s) to Your Science Degree
- Late Graduation Application Form - Winter (March 2027) Convocation
- Request to Graduate from a Different Subject Area or Different Degree
