Updated Billings & Collections Forms

The following forms have been updated for better usability and standardization. Common changes include conversion to a streamlined PDF template, standardized language, and updated submission instructions. In addition, the authorization approval has been changed to align with budget accountability. 

  • Billing Unit Setup and Update Request Form : Approval Authorization has transitioned to the College/Faculty General Managers (FGMs) or Associate Vice Presidents/Strategic Initiative Officers. Finance Partner authorization no longer required. Completed forms must be submitted directly via the University Service Portal.The form will  require a dedicated billing inquiry email address.   • Petty Cash and Honoraria Custodian Authorization Form: Approval authority has transitioned to College/Faculty General Managers (FGMs). Budget Owners, or AVPs/Strategic Initiative Officers, and Finance Partners authorization is no longer mandatory. The updated form now formally includes Honoraria Custodians alongside Petty Cash Custodians. To request this form, please reach out to the Cash Management Office (CMO) via the University Service portal.
  • Cash Deposit Book Request Form -. Authorization approval has transitioned to the  College/Faculty General Managers (FGMs) or Associate Vice Presidents/Strategic Initiative Officers. Finance Partner authorization no longer required. The completed forms must be submitted directly via the University Service Portal. The updated form requires your official @ualberta.ca email, Department ID, and the specific number of books requested. Cashier Deposit Book Request 
  • Forms will be available to align with the implementation date. Ensure you are using the latest versions of all Finance & Distribution forms by searching the Finance, Procurement and Planning Forms Cabinet. Some forms now include dynamic features that adjust based on your responses.
