Incident Lessons Learned: Damaged Universal Power Adaptor

Ensure safe use of power bars and outlets to prevent fires and damage to equipment.

Summary

U of A electricians responded to a work order concerning exposed electrical components. A Universal Power Adaptor had melted while plugged into an electrical outlet. This situation posed a significant risk for serious electrical injury.

The adaptor had been removed from the 120 volt receptacle before the electricians arrived. Upon inspection, there were no visible safety standards identified on the adaptor and it was unclear what device, if any, had been connected to the adaptor when it melted. In response to the incident, the electrical team replaced the melted power adaptor with a safety approved adaptor.

Consequences

Melted power adaptors pose serious risks of electric shock and fire and can cause permanent damage to the connected device. The melting is a sign of a severe electrical issue, such as an overloaded circuit, poor connection, or faulty wiring, which require immediate attention to prevent injury or property damage.

The Culture of Care Safety Action Plan calls for all employees to take responsibility for their own safety and that of others. With that in mind, always ensure your own safety in every situation, including when conducting work where electrical systems are involved.

Call to Action

An overburdened electrical outlet with multiple plug extenders and adaptors connected simultaneously is an example of hazardous electrical misuse. To reduce the risk of fires and other incidents:

  • Inspect cords and plugs regularly for damage and replace them if needed.
  • Ensure plugs fit snugly into outlets, and replace any that are loose or damaged.
  • Avoid overloading outlets — do not plug too many devices into one power strip, and use power strips for low-voltage electronics only.
  • Charge devices in a cool, dry environment on a hard, flat surface.
  • Plug large, permanent electrical equipment (such as refrigerators) directly into a wall outlet, not into an extension cord or power strip.

All work that involves repairing, replacing or otherwise maintaining electrical systems must be conducted by a competent individual. This means someone who is trained, has sufficient experience, and is qualified. In general, this means calling a certified journeyman electrician, so a maintenance request or work requisition should be submitted to the Operations team.
