Forms for Master's Degrees
Master's Degree – Project-based
- Verify that you have completed all the requirements for the project track by consulting this page and ensuring that all the required elements are fulfilled.
- Project formatting: Refer to this document for project formatting
- Research Project Acceptance Form (please make a copy of the form and do not fill it in directly)
- Report of Completion of Course-Based Master's Degree (please fill in all sections without exception)
- If you have completed the four courses of the Embedded Certificate in Leadership, a specific form must be completed that can be found here
- Apply for your graduation ceremony invitation by following this link
Note: The Embedded Certificate is awarded at graduation, at the same time as your final degree.
Master's Degree – Thesis-based (Science, Arts and Education)
- Supervisory Committee Form: The supervisor must check with their student that the Supervisory CMT form has been sent to the Bureau des Études supérieures. If necessary, it must be resubmitted as soon as possible.
- Five weeks before the defence, the supervisor sends the Notice of Examining Committee & Examination Date (Master's Final) form to the ES office.

You have successfully defended your thesis and your examination committee has recommended that you ‘pass’ or ‘pass with revisions’. What now?
1. Submit end-of-program documents:
- Thesis Approval/Program Completion. You must tick the boxes in each section without exception to confirm that you have completed all the requirements for the Master's degree with thesis.
- The deadlines for submitting this document, in order to avoid fees for future sessions, are detailed here.
- University of Alberta THESIS/DISSERTATION NON-EXCLUSIVE LICENCE ;
- Send them to the office no later than six months after your defence (to: csjsup@ualberta.ca).
- If these documents are not sent within six months of passing the defence, the student will no longer be considered to have passed according to GPS regulations (check this same link for the procedure when the thesis requires modifications or is not passed).
2. Register and submit your thesis
- Go to Thesis Deposit, using the following procedure - Submitting Your Thesis Electronically in Thesis Deposit
- GPS will re-read the thesis to verify that it meets the necessary criteria. Wait for GPS to approve the formatting of the final version of your thesis.
- If GPS accepts the thesis, it will send you an email confirming that your thesis has been accepted.
- Your thesis will then be published on the ERA platform and made available to the public free of charge as soon as possible thereafter.

Certificates are issued by the Faculty by mail only to the student's address as indicated in BearTracks. There is no graduation ceremony for postgraduate certificates.

Documents to be provided in the first year
Supervisor First Meeting Checklist
This form is automatically sent by email via GPS to the supervisor first. Please do not fill it out or attach it to an email to send it to us. It will be sent to you automatically by the FGPS via email.
This form is an official document governing the relationship between a student and their research supervisor at the University of Alberta.
In summary, its purpose is to:
- Clarify mutual expectations between the student and the supervisor, particularly regarding roles, responsibilities, deadlines, and objectives of the research program
- Formalise the agreement on the course of study (courses, research, ethics, meetings, publications, funding, safety, confidentiality, professional development, etc.)
- Ensure harmonious and respectful supervision by promoting open and transparent communication throughout the program
- Serve as an official reference in the event of questions or disputes between the student and the supervisor
The supervisory committee: at least three members before the end of the first year.
- The supervisor
- Professor in the relevant field of research
- One other professor in the relevant field of research
Ensure that their qualifications and the composition of the committee are appropriate.
This committee must be formed before the end of the first year of admission.
The form (Appointment of Supervisor and Supervisory Committee) must be sent before the end of the first year of admission.
Note: This committee must hold a formal and regular meeting with the student at least once a year.
