Program Administration

  • Course Outline Template - Graduate (Word) Graduate course outline for faculty instructing graduate courses Fall/Winter/Spring/Summer.
  • Graduate Student Annual Report Each year students are asked to complete an annual report of their activities over the previous year. The form document's student's progress in the previous year and plans for the coming year.
  • Directed Studies Request (PDF) DEADLINE: at least one week prior to the registration deadline for the term of registration. Refer to the University Calendar for term registration deadlines.
  • Capping Project (KSR 900) Registration Form (PDF) DEADLINE: at least one week prior to the registration deadline for the term of registration. Refer to the University Calendar for term registration deadlines.
  • Graduate Student Ethics Training - register on Beartracks in INT D 710: Ethics and Academic Citizenship (both Master's and Doctoral students) and INT D 720: Advanced Ethics and Academic Citizenship (for Doctoral students)
  • Individual Development Plan & Professional Development Completion form is completed and on file with the Graduate Programs Office

Committees, Exams and Program Completion

  • Appointment of Master's Supervisory Committee (Thesis-Based)
    Form to appoint master's thesis-based students' supervisory committee by the end of the first year of the student's program.
  • Appointment of Academic Advisor and Coach Mentor (Course-Based)
    Form to appoint master's course-based students' academic advisor and coach mentor (if applicable) at the time of admission.
  • Working Agreement (Masters and PhD students)
    Form to be completed by the Graduate Supervisor at the time of the thesis proposal meeting (Master's and PhD theses).
  • Approval of Thesis for Examination (Masters students)
    This form is to be completed and retained in the student's file at the time that the Notice and Approval of a Master's Final Oral Examining Committee & Examination Date form is completed and sent to the Faculty of Graduate Studies and Research (FGSR).
  • Final Doctoral Examination Preliminary Acceptance of the PhD Thesis
    Form to completed by the student's supervisor and supervisory committee to declare that the thesis is of adequate substance and quality to warrant that the student proceed to the final examination, or that the thesis is unsatisfactory and the student should not be allowed to proceed to the final examination.
  • Notice of Completion of Capping Exercise (KRLS 900) (Masters Course-Based students)
    Form for student's supervisor and second reader of the Capping Exercise to declare the student has successfully completed the requirements of the capping exercise in the Faculty of Kinesiology, Sport, and Recreation as set out by his/her academic advisor.
  • Report of Supervisory Committee Meeting (Thesis-Based)
    There is an expectation that an annual meeting of the supervisory committee held with each student. The performance of the study should be determined and specific suggestions for improvement and/or activities to be included which a specific time frame identified.
  • Thesis Guidelines
    Master's and Doctoral thesis guidelines specific to the Faculty of Kinesiology, Sport, and Recreation. Sample formats for Traditional and Paper based theses are included.
  • Capping Exercise Guidelines
    Master's (course-based) guidelines for completion of the Capping Exercise: KRLS 900. Three primary ways for MCoach, MA(RLS), and MA course-based students to complete their Capping Exercise are included.

  • Graduate Students' Association (GSA) Assistantship Time Use Guideline
    This form must be completed by the Assistantship Supervisor and graduate student at the start of each University Term (excluding Faculty Operation Funded Spring/Summer RA's where the Report of Completed Activities is required 30 days after the last month of the assignment)
  • Graduate Studies Spring/Summer RA Report of Completed Activities
    The Report is due 30 days after the last month of your Spring/Summer RA assignment (Faculty Operation Funded Spring/Summer RA's ONLY).
  • Graduate Student Teaching Development and Resources (PDF)
