Refer a Patient
The Oral Health Clinic offers dental services at reduced rates. We are now accepting new patients and referrals. Dentists and physicians can refer patients using the forms below.
Endodontics
We accept referrals from private practitioners for select endodontic treatment in our undergraduate program.
Periodontology
We accept referrals from private practitioners for the management of periodontal diseases and the placement of dental implants.
Oral Medicine
We accept referrals from private practitioners for oral lesions/pathology and TMD/orofacial pain.
Orthodontics
Patients can self-refer for orthodontic treatment from our graduate program using our general patient intake form.
