Health Faculty Immunization Program for 2022/23

2022/23 Health Faculty Immunizations

Congratulations on acceptance to your program! All first-year health science students are required to complete the 2022-2023 Health Faculty Immunization Clearance Form. We encourage you to initiate this process as soon as possible in order to protect yourself and the patients you will be interacting with during your placement.


Before you begin the process, please ensure that you have gathered all of your immunization records from infancy to childhood. Please note: records should be in English or French - records in any other language must be translated prior to sending them to us. Your “University Health Centre Welcome Letter” contains more information on how to request immunization records. 

Outlined below are the first steps that are required to have your immunization review initiated by the staff at University Health Centre:

  1. Complete the Appointment Request Form for Health Science Students to initiate your immunization review by a registered nurse at the UHC. 
  2. Create a Medeo account.* Medeo is a secure messaging platform used by the UHC to send you the required information and forms needed to complete this process. 
  3. After completing this request form, you will be contacted by a UHC team member with further instructions.

Note: If you have started, have completed or plan to have the COVID vaccine series in the near future, you will need to wait  28 days before receiving a TB skin test. If you require a TST, you can have a COVID vaccination any time AFTER  the TST is read. Please keep this in mind when booking your appointments.


Fees - Please note that provincial health care does not cover these fees.

  • There is an $80.00 fee for Health Sciences Students accessing their first health faculty immunization appointment.
  • If you come to your appointment without complete immunization records, there will be an additional $40.00 fee, for a total of $120. 
  • Students arriving late will be rescheduled and charged a $50.00 late fee. If you need to reschedule or cancel your appointment, ensure that you contact us 24 hours prior to your appointment to avoid the $50.00 fee (no exceptions).
