2020/21 Health Faculty Immunizations

Your chosen program of study includes clinical placements as an essential component of your program. In order to protect yourself and the patients you will be interacting with during your placement, you are required to have your immunization records reviewed and updated by a health care professional. We encourage you to visit us to ensure that your immunization requirements are met.


Reviewing Your Immunizations

Upon acceptance to the program, all first year health science students are required to complete the 2020-2021 Faculty Immunization Clearance Form. It is imperative that this form is initiated as soon as possible, as in the absence of some prior immunizations, the immunization process can take upwards of 7 months to complete.

Important note: The process has been changed for 2020-21 because of the ongoing COVID-19 pandemic and the need to minimize the number and length of in-person appointments. Outlined below are the first steps that are required to have your immunization review initiated by the staff at UHC:

  1. Gather all available immunization records (from infancy to adulthood). Please note: records should be in English or French. Records in any other language must be translated. If your immunization records are located outside of Canada, check with your family, physician, public health unit, or school in your country of origin. If you immigrated to Canada as a child, Immigration Canada requires an immunization record signed from a physician in your country. Ask your parents if they have a copy of the form.
  2. Create a Medeo account. Medeo is a secure platform used by UHC to communicate with patients related to their health care needs.
  3. Complete the Appointment Request Form for Health Science Students to initiate your immunization review by a registered nurse at UHC. If you are a Medicine Student, please review and submit this form.
  4. After completing this request form, you will be contacted by a UHC team member with further instructions.

Please be aware that this service is not is covered through provincial health care. There will be an associated fee of $80.00 ($120.00 if you are unable to provide acceptable immunization records or documentation) for all health science students except for students in Medicine. (Medicine has additional requirements with a fee of $100/$140, depending on whether or not you can provide acceptable immunization records.)

Fees

  • There is an $80.00 fee for Health Sciences Students accessing their first health faculty immunization appointment; a $100.00 for the AFMC Form will apply to students in Medicine.
  • If you come to your appointment without complete immunization records, there will be an additional $40.00 fee, for a total of $120 ($140.00 for AFMC Form for students in Medicine). Please note that the provincial health care does not cover these fees.
  • Students arriving late will be rescheduled and charged a $50.00 late fee. If you need to reschedule or cancel your appointment, ensure that you contact us 24 hours prior to your appointment to avoid the $50.00 fee (no exceptions).

NOTE: Effective as of June 1, 2015, there is an additional $20.00 administrative fee for any student requiring a dTap booster during the appointment. This fee must be paid regardless of where you previously completed your immunization form.
